Before

This lady in her 60's presented with very hard, distorted, and uncomfortable breasts. Her complex history included breast augmantion in the 1970's followed by three subsequent surgeries to try to fix hardening of her breasts due to contracture of the scar capsule around her implants. On at least one occasion, implants covered in polyurethane foam (Meme) were used but nothing worked. In 1991, her implants were moved from above to under the chest muscle in hopes of them staying soft. This also failed. Her medical history included insulin dependent diabetes, liver cirrhosis, and chronic back pain for which she regularly vaped cannabis oil. In short, she was a mess and not the best candidates for further implant surgery. She wanted to feel better and to have her breasts look as normal and nice as possible. After obtaining medical clearance from her primary physician, I performed an extensive revision of her breasts.

She is shown here 7 months after her breast revision. I removed the old implants which were both ruptured. These were Allergan textured, round silicone gel implants, 300 cc in volume. I also removed all of the old scar around them, which was very thickened. In each breast, I found a hard disc of calcium above the muscle, which I removed. This was almost certainly calcified scar from her old, above the muscle implants. I place new implants: Allergan Natrelle Inspira Style SRM smooth, round silicone gel implants,375 cc in volume. I also placed into each pocket a sheet of BellaDerm, which is banked human dermis (skin) to try to prevent future contracture and hardening. She did beautifully with the surgery and, 7 months later, has breasts that look dramatically better and are very soft. I sent her scar tissue to be checked and it was found to be just that, scar. She is happy and I am thrilled. Her old implants the scar tissue around them are shown.
