Before

This lady had breast augmentation with textured silicone gel implants in 1991. She did well and was happy with her result but admitted that her breasts became firmer over the years. In 2017, she was diagnosed with cancer in the left breast and underwent a lumpectomy with radiation therapy to the breast. After this, the left breast became even firmer. She had an MRI in 2019 that showed intracapsular rupture of both implants. She came to me to exchange these implants for new ones. She did not want to go without implants. Her overall health was excellent. 

After

I removed her implants and the entire capsule around them. The capsules were not terribly thickened but they were very fibrous and contracted, the left more than the right. Her left implant was intact but the right implant had leaked. The gel was confined to the implant pocket. I replaced her implants with a pair of 325 cc Mentor MemoryGel smooth, round, moderate plus profile XTRA implants. The XTRA implants are slightly overfilled to reduce the occurrence of visible or palpable rippling. After surgery, I put her on a protocol of Doxycycline and Montelukast for three months to prevent recurrence contracture. She is shown 6 months after surgery with very soft breasts. She is slightly uneven because of her cancer treatment. Her implants and capsules are shown. The implants had no identifying markings. Implants today all have the manufacturer and serial No. imprinted on the shell.  Pathology on her capsules showed only scar tissue.

The capsules are fairly typical for old implants. They are red because of blood staining, otherwise, they would be white. The implants are yellowed due to their age
