Before
This lady had breast augmentation in 2004 by a plastic surgeon in Orlando. She had saline implants placed and felt from the start that they were too big. She went from a B to D cup with 390 cc implants that were overfilled to over 400cc. She experienced bottoming out with the implants sitting too low. I saw her in 2017 at which time she asked that the implants be removed. I did this and tightened up the overly stretched out skin on the lower half of her breasts. I noted at that time that the capsules (scar tissue) around her implants was extremely thin and filmy, which is probably why she bottomed out as she did. She was initially happy without implants but returned in 2019 seeking to have implants put back in. I did this for her. At this time, she wanted to go with gel implants.
After
She is shown 6 months after putting in a pair of 330 cc Allergan Natrelle Inspira smooth, round gel implants. I placed these in the same under-the-muscle position as her old implants. I took care to avoid over-extending the lower half of the pocket. Despite this, she has demonstrated some recurrent bottoming out, but to a lesser degree. At this time, she is happy and we are just going to watch this for now. I hope to see her back annually for a checkup
