Before
This patient came in for a deflated left saline breast implant. She underwent breast augmentation elsewhere in 1999 with Mentor Siltex textured, round saline implants. Her implants were 475 cc. The right was filled to 505 cc and the left to 520 cc. She developed a large seroma (collection of serum in the implant pocket) on the left and this was drawn out with a needle. Over the years, her breast became firm and the implants sat too high in her breasts. Her left implant deflated suddenly March 2020 without any obvious cause. I think the seroma contributed to her breast getting hard but do not think the needle aspiration damaged the implant.
After
We got her into surgery right away to avoid the left pocket shrinking and making surgery more difficult. Her left implant was totally flat but the right one was perfectly intact and looked fine. I had to release some fibrous capsule around them to get new implants in. Because of this, and her history of a seroma in the past, I used drains in her, something I rarely do. They were removed at 6 days. Her new implants are Mentor smooth, round, moderate plus profile saline implants with a volume range of 475-570 cc. They were both filled to 540 cc. Her photos were taken just shy of 6 months after the surgery. Her result looks and feels very natural. Notice how the excessive fullness of the upper right breast has been corrected. The implants feel exquisitely soft. She has normal feeling in both breasts. ( I used textured saline implants only briefly in my practice and quickly stopped because I did not like them. They were too stiff and had a very high rate of failure and deflation. They also had much more tendency to show ripples and wrinkles through the skin. )
