Before
I see a lot of women these days who have implants that are 20, 30, and even 40+ years old. Some are doing just fine but some have issues, such as hard, misshapen breasts, or have been found to have implants that are leaking. Some elect to have their implants removed for these reasons, or simply because they feel they no longer need or want them. One of the concerns often expressed is what will their breasts will look like without implants, after all these years with them. This 67 year-old lady had her implants placed in Pennsylvania 46 years ago. She came to see me when her mammogram showed that both implants were leaking. Her breasts were quite firm and somewhat misshapen but her breast exam was otherwise normal and her health was excellent. She decided to have the implants removed and not replaced.
After
When I removed her implants, I found that both had leaked but the silicone was confined to inside the implant pocket. The capsule (scar) around her implants, which all patients form, was very calcified and I removed both capsules completely, a procedure called an "en bloc" capsulectomy. After surgery, she was very pleased with the look and feel of her breasts and happy with her decision to not have new implants placed at this point in her life.
