Before
This young lady presented for breast augmentation. She had two children and breast fed both. Her original bra size was a 34 C, went up to a DD cup while nursing, and now was an A cup. She had also lost some weight, which added to the shrinkage of her breasts. After a thorough discussion regarding implants, including a detailed explanation of all the limitations and potential problems related to them, she chose to proceed.
After
She is shown 6 months after surgery. Her augmentation was done using Mentor smooth, round, moderate profile saline implants. I place them in a dual plane- under the muscle in the upper half of the pocket and under the breast in the lower half. As is my practice, I sized her during the surgery and chose 350 cc implants that I overfilled to 375 cc. Her postoperative size is a 34 D cup from Victoria's Secret. Her breasts are equisitely soft and she has normal sensitivity in her nipples. I can tell from how she has done that she has formed very thin filmy capsules around her implants. This is great in that the breasts remain very soft and feel very natural. It can present a problem over time in that the implant pockets can gradually expand and this can lead to the implants sitting too low when she is upright or shifting too far to the side when she lies on her back. The only preventative for this is to always wear good quality, supportive bras and go braless as little as possible. This is part of the delicate balance with all implant surgery- getting that perfect "Goldilocks" result of not too tight and not too loose, just right. Breast augmentation is an unnatural process and getting perfectly natural results is an outcome that I always strive for, but only rarely achieve.
