Before
This lady presented with a complicated history of having had saline breast implants placed over 20 years ago, which became firm and have remained so. Recently, she was diagnosed with cancer in the left breast and, because it was in more than one site, a simple mastectomy was planned. She desired reconstruction and was felt to be a candidate for a nipple-sparing mastectomy with immediate placement of a silicone gel implant. Because of her previous surgery, I performed both the mastectomy and reconstruction.
After
She is shown 6 months after the surgery. I used a 325 cc Mentor smooth, round, moderate profile implant, which I filled to 375 cc. In retrospect, this might have been slightly on the small side, but she looked very even on the operating table. She did beautifully, with no problems whatsoever. Her pathology report was excellent. She has some contour differences between the two breasts, but this was an issue even before her surgery. He recontructed breast is very soft. This was an ideal situation for a nipple/sparing procedure.
