Patient - Breast Reduction

Before
It is not only women who seek to have their breasts reduced. This gentleman in his 60's was unhappy with his feminized breasts and would not allow his chest to be uncovered, even at the pool. He was in good health but was taking a medication for his prostate that is known to cause male breast enlargement. On of his concerns was scars and wondered if liposuction would fix this. I explained that his age, prior weight loss, and location of this problem made liposuction problematic. I recommended a breast reduction with an incision around the areolae in order to hide the scars on the margin of the areolae. I did this and all went well. I removed about 160 grams (about 5.6 oz) of breast tissue from each breast. The result was not acceptable to either us, so a few months later I took him back to surgery and removed the breast tissue more aggressively. This time I removed the nipple/areola complexes completely and then put them back on as grafts. He did well with this with a much better chest contour and acceptable scars.
After
He is shown after his first surgery. There is still too much fullness to his breasts and neither of us was happy with the degree of tigntening of the chest skin. I offered to redo this with a more aggressive skin removal, which would leave a more visible scar, and grafting of the nipple/areola complexes back onto the chest after removing them completely. He elected to do this.
