Patient - Fat Grafting

This delightful lady in her 70âs has very fair skin and her genetics, age, and accumulated sun damage have created severe wrinkling primarily around her mouth. She also has experienced some loss of fat in her face giving her a more severe appearance which belies her very pleasant, outgoing personality. She did not want extensive surgery. Only the most aggressive peels will substantially improve wrinkling of this degree and I recommended a peel using our carbon dioxide laser. She did not want a facelift- which would not add any volume to her face in any event. She needed more volume than we can practicably give with commercial fillers such as Juvederm or Radiesse. Sculptra is expensive and lasts a few years. Artefil is permanent but I hesitate to use permanent artificial fillers (see my blog on fillers) so I suggested fat grafting.

The peel and fat grafting were done a few months apart. A full correction was not anticipated, nor would it be desirable. If the skin around the mouth were too smooth, it would look odd against the rest of her face. I would have liked a little more fullness from the fat grafting but her age may have been a factor in how much of the grafted fat survived. Even so, she has a much more youthful, pleasant, and soft looking face with she is very pleased and so am I.
