Patient - Neck Lift

Sometimes a patient comes in who requires thinking outside the usual box. This gentleman was unhappy with the loose wattle of skin under his chin. He really did not want major surgery. Most neck tightening is done by means of a full facelift, but this is a big operation. One option that I consider under unusual circumstances is a direct excision neck lift. This involves removing skin directly from the neck instead of doing with incisions around the ears. The advantage is simplicity and focusing only on the area of concern. The downside- a non-starter for some patients- is the scar on the neck. To avoid a constricting, band-like vertical scar on the anterior neck, this is done using a 'zig-zag' incision that breaks up the scar line so it cannot contract and pull on the neck or create a tight vertical band. This also makes the scar less conspicuous. I proposed this operation to him.

He is shown 8f months after the surgery. The improvement is obvious and, I feel, very acceptable. He is happy and so am I. I doubt that even a full facelift would have done more for his neck because his skin excess and looseness were so severe.
